Many CEOs Of Health Industry Groups Earned Big Bonuses

The CEOs of many major healthcare trade groups earned big bonuses in 2009 as congressional debate on healthcare legislation dominated their agendas.

Overall, CEOs were well paid: Nine out of 12 professional health care organizations paid their CEOs compensation of $1 million or more, including bonuses, deferred compensation, and other benefits, according to 2009 tax filings. (Exempt groups must file an annual application with the IRS on a publicly available Form 990. The data is typically one year old.

“This was an industry that faced a bet-on-the-farm debate, which is why you see these executives getting the big bucks,” said Ivan Adler, a director at headhunting firm McCormick Group.

Most of these compensation packages were approved by trade association boards in 2008, before the great healthcare battle, and salary levels for some executives were lower in 2009 than in 2008. It is becoming more common for these packages to include a base salary plus a bonuses and deferred compensation to incentivize key association personnel to achieve financial, advocacy and other association-specific goals.

“There is no formula [for] association compensation,” said Nels Olson, managing director of headhunting firm Korn/Ferry. “But one that’s relevant here is advocacy in light of the health debate in Washington.”

Tauzin, the former CEO of PhRMA, is among those who received a bonus. In addition to his base salary of $2.1 million, he received a bonus of $2.3 million. James Bryant Hall, PhRMA’s top federal lobbyist after Tauzin, received a $91,750 bonus on top of his $371,182 base salary. Another major healthcare player, Blue Cross and Blue Shield Association CEO Scott Serota earned a $1.6 million bonus on a $856,055 base salary, and BCBSA chief lobbyist Alissa Fox earned a $130,000 bonus US dollars plus a base salary of US$278,760. Federation of American Hospitals CEO Chip Kahn received a $900,000 base salary and a $315,000 bonus, while Jeff Cohen, one of the group’s top lobbyists, received a $325,000 base salary and a $315,000 bonus Earned $65,000.

PhRMA was among the advocacy winners when the law was passed in 2010. The group agreed to reduce the cost of branded prescription drugs for seniors affected by the “donut hole,” a compromise that would cost the industry an estimated $80 billion. In exchange, the industry received concessions from the White House, which included a promise that the law would not allow cheaper drugs to be re-imported from Canada. Hospitals were also winners. Health insurers have had a mixed record. They successfully repelled efforts to include a public health insurance option and could attract millions of new customers if nearly all Americans get coverage. To be fair to the executives and lobbyists in the accompanying chart, there are a few important caveats. They didn’t get all that money in their paychecks. Deferred compensation programs are financial incentive packages that vest over time but still require annual reporting to the IRS. For example, former AARP chief Bill Novelli’s compensation in 2009 included a base salary of $345,243; a payment of US$1.2 million, which included deferred compensation from his 8-year tenure at AARP and a US$350,657 severance payment; $35,362 in retirement and deferred compensation and $4,209 in tax-exempt benefits. Novelli left AARP in April 2009 and was replaced by A. Barry Rand, whose total compensation was $648,640.
